All HIV Patients Should Take Meds Early On, Experts Now Say ...

MONDAY, July 23 (HealthDay News) ? A new report recommends that physicians begin treating all HIV-positive adults with antiretroviral drugs instead of waiting, at least in countries where the therapy is easily accessible.

All HIV-positive patients should take the drugs, even if blood tests reveal that their immune system is healthy, according to the revised guidelines, which were presented Sunday at the International AIDS Conference in Washington D.C., and published in the July 25 issue of the Journal of the American Medical Association.

Over the last 25 years, antiretroviral drugs have become stronger, easier to tolerate and simpler to take, the authors of the report explained. ?New trial data and drug regimens that have become available in the last two years warrant an update to guidelines for antiretroviral therapy in HIV-infected adults in resource-rich settings,? Dr. Melanie Thompson and her 2012 International Antiviral Society-USA Panel colleagues said in the report.

While medications don?t rid the body of HIV ? the virus that causes AIDS ? they can prevent the virus from replicating and have allowed millions of people to live for years with a greatly reduced threat of developing AIDS, experts have noted.

However, not every person infected with HIV takes the medications. In some cases, doctors prefer to wait until a patient?s immune system begins to show signs that the virus is becoming stronger.

The reasons why doctors sometimes wait to prescribe antiretroviral medications may be because the drugs can cause side effects, and there?s also the challenge that patients must keep taking them or take the risk that the virus will rebound.

Commenting on the report, Rowena Johnston, director of research with the Foundation for AIDS Research in New York City, explained, ?We can?t lose sight of the fact that this is a big commitment. Once you start, you can?t stop.?

The report recommends that initial treatment include two nucleoside reverse transcriptase inhibitors (tenofovir/emtricitabine [brand name Truvada] or abacavir/lamivudine [known as ABC/3TC]) plus a non-nucleoside reverse transcriptase inhibitor (efavirenz [Sustiva]), a ritonavir-boosted protease inhibitor (atazanavir [Reyataz] or darunavir [Prezista], or an integrase strand transfer inhibitor (raltegravir [Isentress]).

The new guidelines are a sign of a ?turning point? for HIV drugs, Johnston said. These medications are now being considered not only as a treatment but as a prevention tool because those who take them are less likely to infect other people, she noted. ?The scientific community really recognizes how valuable they could be to prevent transmission,? she said.

There are challenges, though, Johnston pointed out. One obstacle is money to pay for the drugs, even in rich countries such as the United States. There?s also the matter of getting people to be tested in the first place and then to seek and retain medical care if they learn they are HIV-positive, she said.

DETROIT (AP) ? Miguel Cabrera's 300th homer cleared the wall in center field with room to spare and sailed toward the ivy at Comerica Park.

That's not the easiest place to retrieve a baseball, but this one should make a nice keepsake.

"Somebody got it. They got it somehow," Detroit manager Jim Leyland said. "They got in there and got it, and that's his 300th, so I'm sure they'll take care of it for him."

Cabrera homered twice to reach another milestone in his remarkable career, and the Tigers finished a three-game sweep of the Chicago White Sox with a 6-4 victory Sunday. Detroit wrapped up a 6-1 homestand against the White Sox and Los Angeles Angels and now leads the AL Central by 1? games over Chicago. The White Sox have lost five straight.

Cabrera became the second Venezuela-born player to reach 300 home runs. Andres Galarraga hit 399.

"For my country, yes, it's very special," Cabrera said. "Hopefully a lot of players can follow from Venezuela and try hitting 300 more."

Quintin Berry and Brennan Boesch also homered for Detroit to help rookie Jacob Turner (1-1) earn his first career win. Four relievers pitched for the Tigers, with Joaquin Benoit working a perfect ninth for his second save of the year.

Detroit closer Jose Valverde has been dealing with a balky back and did not pitch.

Philip Humber (4-5) allowed six runs in three innings. Alex Rios and Kevin Youkilis homered for the White Sox.

The Tigers have won 16 of 21.

Turner, Detroit's rookie right-hander, bounced back after giving up seven runs in two innings in his last start against the Angels. He went 5 1-3 innings, allowing three runs and seven hits. He struck out three without a walk before being lifted after Rios hit a two-run homer in the sixth.

"I think the biggest difference is just being more aggressive, attacking the hitters and not really worrying about who might be at the plate, but just going right after them," Turner said.

Cabrera and the Detroit offense provided plenty of support. After an RBI single in the top of the first by Rios, the Tigers scored three runs in the bottom half before Humber retired a batter. Berry's two-run homer gave Detroit the lead, and Cabrera followed with a high drive that cleared the fence just to the left of center field for home run No. 299.

"I picked the wrong day to get balls up in the zone," Humber said. "The stuff was there, but when you are facing that team, and especially that team when they are hot, your stuff doesn't matter if you leave it up."

Cabrera's second homer, in the third inning, was even longer. It ended up around where a camera platform is set up beyond the wall in center. A Tigers spokesman, citing ESPN Stats and Info, said it was estimated at 457 feet.

"Dude has some of the most ridiculous pop I've ever seen," Berry said. "His smooth swing, to be able to go that high up and dead center, it's amazing. It's fun to watch. He does it in BP and he does it in the game. It's unreal."

It was Cabrera's 23rd homer of the year, and he became the 14th player to reach 300 before turning 30, according to STATS, LLC. Cabrera was 29 years, 95 days old Sunday. When Hank Aaron was that age, he had 309 home runs, according to STATS. Barry Bonds had 222 by that age.

Boesch added a two-run shot later in the third to make it 6-1. Humber allowed seven hits and a walk with no strikeouts. He's 3-5 with a 7.55 ERA since pitching a perfect game at Seattle on April 21.

The White Sox chipped away after Humber was taken out. Rios' hit was ruled a homer after umpires consulted replay footage, and Youkilis hit a solo shot off Phil Coke in the seventh.

Octavio Dotel, the third Detroit reliever, came on with one out and a man on in the seventh and retired all five hitters he faced, with four strikeouts.

The White Sox were swept in a series for the first time all season.

NOTES: Coke was pitching for the first time since his wife gave birth to a baby girl. Mickenzie LouAnn was born Saturday morning. ... The Tigers could afford to go deep into their bullpen with an off day coming up Monday. Chicago hosts Minnesota, with RHP Gavin Floyd (7-8) returning from a problem with his throwing elbow to start against Minnesota LHP Francisco Liriano (3-9).

East Shore Twilight Baseball League president Steve Trees quits

East Shore Twilight Baseball League president Steve Trees resigned Friday night, the result of a scheduling dispute about a makeup doubleheader slated for Saturday afternoon that grew testy.

Trees had been EST president since 2003. He has been involved with the sandlot circuit as a player for Lawnton, and later as a coach and administrator, since 1978.

A scheduling dispute between Middletown and Dauphin arose when the Brewers requested a move of Saturday's second game (a weather-related makeup game not on the original docket) earlier this week because they would only be able to field eight players.

Citing league bylaws that he said govern makeup games differently (both teams must agree on time and site), Trees ruled that the makeup game would be moved again -- this time, to July 22 at Dauphin -- when the two clubs couldn't come to an agreement on another date and location.

Reached Saturday, Dauphin manager Jay Malehorn admitted that he vigorously protested the move to Trees. Trees said he resigned in response, saying his frustration level in running the league had been "building for some time" and that Friday's incident was the final straw.

Saturday's originally scheduled game between Middletown and Dauphin went off without incident. Dauphin won 3-0.

Trees said league treasurer Joe Pomone would take over, effective immediately.

HTC Droid Incredible 4G LTE (Verizon Wireless)

No, this isn't one of the HTC 'One' Phones. Rather than keep the One design and slap a new name on it, like Sprint mostly did with the HTC EVO 4G LTE?($199.99, 4 stars), Verizon and HTC are going for something different here. The $149.99 HTC Droid Incredible 4G LTE is more of an honest-to-goodness follow-up to the HTC Droid Incredible 2?(3.5 stars) than it is an extension of HTC's top-tier new One brand. That's both good and bad.

The Droid Incredible 4G LTE retains the same general shape and size of its predecessors, which is great news for anyone that isn't keen on the super large, hand-busting new smartphone trend. On the other hand, the Incredible eschews some of the best features from the One line, like a faster 1.5GHz processor and HTC's ImageSense camera processor. The Droid Incredible is still a very good smartphone?it's just a notch below the One.

Design and Call Quality
The Incredible 4G LTE measures 4.8 by 2.4 by 0.5 inches (HWD) and weighs 4.8 ounces. It might sound silly, but size is probably the primary reason to get this phone. While other top smartphones tend to rely on the idea that bigger is better, the truth is, phones with a 4-inch display are just more comfortable to hold if you have average size hands. The Incredible is a little thick, but it has a solid feel, with a textured, rubberized black panel. The glass front of the phone is surrounded by a thin, shiny black plastic band, and there are red accents around the camera sensor, on the Power button, and on the speaker grill for the earpiece. All told, it looks a lot like the original Droid Incredible?(4.5 stars).

The 4-inch qHD Super LCD features 960-by-540-pixel resolution. That's not quite on par with the brilliant 720p display on the HTC Rezound ($199.99, 4 stars), but it still looks sharp and bright. And despite the phone's smaller-than-average stature, typing feels fine on the onscreen keyboard.

The Incredible is a dual-band EV-DO Rev. A (850/1900 MHz) and single-band LTE (700MHz) device with 802.11a/b/g/n Wi-Fi. Data speeds are excellent on Verizon's 4G LTE network, which recently led the pack in our testing for the?Fastest Mobile Networks. Download speeds averaged close to 10Mbps on the Incredible, while upload speeds were around 8.5Mbps. You can use it as a mobile hotspot to share that data with up to 10 connected devices with the appropriate plan. And if you're looking to conserve data, the Incredible connected to my WPA2-encrypted Wi-Fi network without a problem.

Voice quality is a strong point. Voices sound loud and clear in the phone's earpiece, if a bit digitized. Calls made with the phone are equally easy to hear, with a natural quality and good noise cancellation. Calls were fine through a Jawbone Era?Bluetooth headset ($129, 4 stars) and voice dialing worked well over Bluetooth. The speakerphone sounds okay, and volume goes loud enough to use it outdoors. The 1700mAh battery was good for a solid 8 hours and 43 minutes of talk time.

Hardware, OS, and HTC Sense 4
The Incredible uses the same dual-core Qualcomm Snapdragon S4 chip as the HTC One X?($199.99, 4.5 stars), except here it's clocked at 1.2GHz instead of 1.5GHz. That still makes for fast mobile performance?just not quite as fast as the One phones or the Samsung Galaxy S III ($199.99-$249.99). Still, you'll be able to run all of the apps and games available in the Google Play store without a problem.

Android 4.0.3 is on board here, along with HTC Sense 4.0. Sense 4.0 is a much lighter implementation of HTC's UI overlay than we've seen before, and very enjoyable to use. You get seven customizable home screens to swipe between. They come preloaded with some helpful apps and widgets, along with a hefty 15 items of bloatware from Verizon. None of the bloatware is deletable, but it didn't seem to impact performance.

NASHVILLE, Tenn. (AP) ? The members of country group Little Big Town are hoping their new song is a hit ? on TV.

They co-wrote and recorded the theme song for ABC's new show "Good Afternoon America." The song is called "Good Afternoon." The nine week summer series starts Monday (2 p.m. ET), anchored by Josh Elliott and Lara Spencer.

Little Big Town singer Karen Fairchild calls it an unexpected opportunity and says the band loved getting the chance to write and record a song for the show.

They'll perform the tune on air Aug. 2 along with their hit single "Pontoon."

The group also includes Jimi Westbrook, Kimberly Schlapman and Phillip Sweet. They are known for hits such as "Boondocks" and "Little White Church."

The Big East landscape is about to change in a big way, and it will be interesting to see which team can rise to the top. This is an opportunity for the UConn football program. Can they take advantage?

Paul Pasqualoni certainly wasn?t happy with a 5-7 record in his first year at Connecticut. The losing record ended a string of four straight seasons with a bowl appearance for the Huskies. The team was close, but it just couldn?t finish the deal in 2011. The Huskies lost four games by a touchdown or less. They?ll need to execute better down the stretch this season.

Team Strengths:

Running back is definitely an area of strength for UConn. Lyle McCombs is only 5?8, but he averaged 95.92 yards per game on the ground in 2011, which was second best in the conference. McCombs did all of that in his freshman season, so expectations are high for him this year. There isn?t a lot of depth behind him, so the Huskies need McCombs to stay healthy.

The front seven on defense should be strong once again this year for the Huskies. Trevardo Williams is one of the best pass-rushers in the nation, and I expect him to wreak havoc on many opposing teams this year. All three linebackers return from last year, so the leadership will come from that unit.

Team Weaknesses:

A few years ago the strength of this Huskies offense was their offensive line, but now it is their single biggest concern heading into 2012. Two of their top players graduated, and three more suffered injuries in the offseason. The line is going to be extremely short on experience, and that could be a major problem all season long.

The quarterback competition is on once again in Connecticut this year. Johnny McEntee was the man last year, but don?t be surprised if junior college transfer Chandler Whitmer ends up under center. No matter who is the starting quarterback for UConn, there are lots of question marks moving forward.

2012-2013 UConn Huskies Football Prediction: 5-7

This is a case where the Huskies will likely be a much better team than last year, but they might not end up having anything to show for it. They travel to Rutgers, USF, and Louisville inside the Big East. The offensive line will be a major problem this year, and the defensive line won?t be as dominate as it was in 2011. There will be some progress here, but it likely won?t show up in the standings just yet. I?ll take the Huskies to finish just below .500 once again.

NEW YORK (AP) - Ryan Dempster is too busy watching "Swamp People" to see the stories about him being possible trade bait. He's pitching too well, though, to know that he might be in another uniform by the end of the month.

Dempster extended his scoreless innings streak to 27 in his first start in three weeks, Starlin Castro hit a three-run homer and the Chicago Cubs beat the New York Mets 7-0 Sunday.

"I don't really notice a lot of the rumors," Dempster said. "I just handle what I can handle and that's going out there and pitching and doing my job. Wherever the cards fall they fall."

Dempster (4-3) was activated from the disabled list then pitched five innings of four-hit ball in his first outing since June 15, when he experienced tightness in a back muscle. His lengthy string of zeros is the Cubs' best for a starter since Ken Holtzman went 27 innings in 1971.

The Cubs made it as comfortable as possible for Dempster, scoring four runs before he even threw a pitch.

Alfonso Soriano had an RBI single, Jeff Baker a run-scoring double with a jam shot to the opposite field and Geovany Soto drove in two runs with a single against Jonathon Niese.

Niese (7-4) had only given up four or more runs in a game five times - 16 starts - this season entering Sunday.

"They came out swinging," Mets manager Terry Collins said. "He was trying to get ahead (and) they were hammering the first pitch."

The Cubs finished the first half in the midst of a modest surge, winning nine of 13 to improve to a disappointing 33-52 in Theo Epstein's first year in charge. It's their worst record before the break since 2006 when they were 34-54.

The team has been a little up and down but everybody hustles right now," Castro said. "Let's see in the second half everybody pull a little bit together and win more games."

The Mets had a chance to move a season high-tying eight games above .500 but continued a recent trend of trading wins and losses. They go into the All-Star break 46-40, their best first half since 2010 when they were 48-40. New York finished a homestand against last place teams, Philadelphia and Chicago, 3-3.

Not that the Mets didn't have had their chances Sunday. They put runners in scoring position in each of first three innings and also in the sixth but came up empty.

Dempster put his streak in jeopardy by allowing a one-out triple to Scott Hairston in the second but he got Jordany Valdespin to line out to second and Josh Thole to groundout to shortstop. Niese and Ruben Tejada opened the third with singles but the Nos. 2-3-4 hitters couldn't capitalize.

After Tejada's hit, Dempster retired nine in a row before being lifted.

"He was way more efficient than we thought," Cubs manager Dale Sveum said. " Everything went well. He feels great."

The right-hander was scheduled to start the Cubs' first game after the All-Star break on Friday but Sveum said Dempster will start Saturday instead to better fit his throwing program. Paul Maholm will move up from Saturday.

Dempster has won four in a row after an 18-start winless streak dating back to Aug. 11. He struck out four and walked none.

Maholm came on in relief for the first time. He made 201 consecutive starts until he entered in the sixth and gave up two hits. But the Mets failed again with runners on. Ike Davis popped to shortstop and Lucas Duda grounded into a double play.

Manuel Corpas, James Russell and Shawn Camp finished the eight-hitter.

Niese had settled in after the first, allowing only one hit until Luis Valbuena singled with one out in the seventh. Reed Johnson singled with two outs and Castro hit the first pitch for a three-run homer, his seventh long ball this year.

"A little tough the last couple of weeks, but today something good happened," Castro said.

Niese allowed seven runs and nine hits in seven innings.

NOTES: To make room for Dempster on the Cubs roster, Chris Volstad was optioned to Triple-A Iowa. Volstad is 0-7 this season. ... Creating an information product is the first thing most people want to do when they start an online business or transfer their offline business online. Why not? Information products have very low overhead, since they?re developed electronically, and other than time for product creation and formatting, they are pretty simple to create.

However, creating a product is NOT the first thing to do in your online business. Why? Because there is a foundation that needs to be constructed before the product creation process begins. You would never start with the roof when building a house, right? The foundation and support beams need to be in place before you can begin work on the roof. The same principle applies with information product development. Too many people develop a product without doing any of the research necessary to determine the need for their product.

Here are the 7 biggest mistakes information marketers make when creating a product:

1. Haven?t chosen a niche or a target market. Trying to sell a product to everyone will never work. It?s only when you target a specific market or a specific niche that you will find success online. It?s so much easier to market to a specific group of people or a specific industry than targeting everyone online. Pick a niche or a target market that is familiar to you, either because you?re passionate about it or because you have experience in that marketplace.

2. Failure to understand the major problems of your target market. What keeps your target market awake at night, either in fear or in joy? By getting a handle on the problems and interests of your target market, you get a much better understanding of what will cause them to buy. Typically, consumers are triggered by three emotions to make a purchase. Will your product save them time, money, or effort? If so, you?re headed in the right direction.

3. Have no traffic to your site. Just because you put up a web site doesn?t mean that visitors will show up en masse. You must have a traffic generation plan in place to ensure that people see what you are offering. How do you generate traffic? With your ezine, blog, article marketing, podcasting, video, radio show, strategic alliances, hosting teleclasses or webinars?and that?s only the tip of the marketing list. People won?t magically find your site ? you have to create a strategy to drive them there.

4. Have no list and no email marketing system. The money is really in the list, provided the list contains members of your target market. When you engage in the traffic generation strategies described above, be sure that you have a way to capture your visitor?s contact info, thus creating your email marketing list. If you capture their mailing address as well, you can take your marketing offline to direct mail.

5. Lack of relationship with your list. People buy from those whom they like, know and trust, whether they?re doing business online or off. However, when you?re online, you don?t have the ability to get to know someone face-to-face at networking or association meetings. So, you have to create that relationship virtually. Put your headshot in your blog and ezine header. Write great content about your specialty in a conversational tone. Share what?s going on in your life in your ezine or on social networking sites. Be helpful and answer questions on industry discussion forums. Visit industry blogs and post relevant comments.

6. Inability to purchase what you?re selling. The worst thing you can do when you?re selling something online is to make it difficult for people to purchase from you. The cleanest and easiest way to make purchasing easy is to get a merchant account and a shopping cart system. You can use Paypal as your payment system, but you have to ensure that it is set up properly so that your customer is able to immediately access the info product she has purchased.

7. Lack of research on competitive products. I think it?s always helpful to research what your competitors are offering and the price points at which the products are being offered. This information can help you determine the market saturation for this info as well as give you pricing guidelines. In fact, you may discover a superior product that already exists, and instead of reinventing the wheel, you decide to become an affiliate marketer for that product rather than try and create it on your own.

Bonus tip: Lack of content. The #1 reason that info marketers don?t create products is because they lack content. When faced with the task of creating a product from scratch, the entire project becomes overwhelming and it falls through the cracks. However, if you?re in the habit of regularly creating new content via the articles you write for your ezine and the posts you submit to your blog, in just a few months you have enough content to easily begin repurposing into products.

Make sure your foundation is solid before proceeding with product creation. By creating a solid foundation first, you have just enhanced your possibility of success by a factor of 10.
